MISSION AND GOALS
NORTH CENTRAL INDIANA CONTINUITY OF CARE
MISSION/PHILOSOPHY: Everyone should have access to an educational and supportive network of professionals in their journey through the health care system.
NCICC exists to provide resources and educational programs to enhance the continuum of care. It provides a medium for sharing professional challenges, resources and feasible solution in the health care environments. It serves to heighten competence and expertise in the delivery of continuity of care by providing professional assistance and informative programs.
GOALS
· to provide an organized structure that will enable NCICC members to work together on problems of mutual interest
· to conduct educational programs
· to serve as a resource to related groups
· to provide channels of communications between the NCICC and affiliate groups
· to promote the purpose of, and membership of NCICC
1. To promote interdisciplinary collaboration throughout the healthcare system.
2. To provide a mechanism for sharing mutual problems and feasible solutions between professionals working with patients.
3. To promote the improvement of inter-hospital and intra-agency communication regarding continuity of care.
4. To promote advocacy for the patient in all settings along the healthcare continuum.
5. To enhance professional competence and expertise in the delivery of continuity of care.
6. To provide continuing educational opportunities and resources for its members.
7. To educate patients about the need to be proactive participants in their own continuity of healthcare.
Continuity of Care Definition: Fletcher et al (1984) distinguish between coordination as “the degree to which various components of care bear a useful relation to each other” and continuity as “the existence of some thread—individual, practitioner, group, or medical record—that bind together episodes of care” (p.409) and notes that they are not easily separable and often used to define one another.
The AAFP (American Academy of Family Physicians) defines “continuity of care” as the process by which the patient and the physician are cooperatively involved in ongoing health care management toward the goal of high quality, cost-effective medical care. (1983) (2003)
